  • Is the purpose of the survey crystal clear?
  • Are all the questions relevant?
  • Are all the questions easy to understand?
  • Have you avoided ambiguity?
  • Are all the questions balanced?
  • Do the potential answers exhaust all options?
  • Are the answer alternatives mutually exclusive?
  • Have you used the same type of scale throughout the entire questionnaire?
  • Have you used a combination of open ended and closed questions?
  • Are the questions asked in a logical order?
  • Are the questions and possible answers linked?
  • Have others reviewed the questionnaire?

If you are able to answer yes to all 12 questions, there is a strong probability that you have optimised your questionnaire in relation to the quality of the answers that you can expect to receive - and you can now focus on distributing the questionnaire and analysing the results.
